Kit Carson County Memorial Hospital

CEO's Message

Hello, let me welcome you to the Kit Carson County Health Service District. Where “We Treat You Like Family”.

Allow me to pledge to you that whether you choose our hospital, one of our fine family practice clinics, our home health agency, hospice, or our rehabilitation facility for the delivery of your healthcare needs, rest assured that we see the relationship of the caregiver and the patient as a partnership. Permit me to share a brief overview of Expectations, Rights, and Responsibilities.

High Quality Care: Our first priority is to provide you the care you need, when you need it, with skill, compassion and respect. Tell your caregiver if you have concerns about your care or if you have pain. You have a right to know the identity of doctors, nurses, and others involved in your care, and you have the right to know when they are students, residents or other trainees.

A Clean and Safe Environment: We work hard to keep you safe when you are in our care. We follow special policies and procedures in an effort to avoid mistakes and to keep you free from abuse or neglect. If anything unexpected and significant happens while in our care, you will be told what has happened and any resulting change in your care will be discussed with you.

Involvement in Your Care: We invite your involvement in your care. Please read all consent forms carefully. If there is anything you do not understand ask for an explanation.

If at anytime you do not understand the following, feel free to ask your caregiver to explain.
· The risks and benefits of your treatment, including any long term effects such treatment may have on your quality of life.
· What you and your family will need to do, if anything, following treatment
· The estimated cost of the service.

At times we will need from you accurate and complete information to help us to make good decisions about your care. Please tell us about past illnesses, surgeries, hospital stays, and allergic reactions. Also, tell us about any medications or dietary supplements you may be taking. If you can tell us about any network or admission requirements that your health plan may have, we may be able to help you to realize if you meet those requirements.

Help us to understand who should make decisions for you when you cannot. If you have signed a health care power of attorney, or a “living will” or “advance directive” give copies to your doctor, your family, and to your care team.

Protection of your privacy: We respect the confidentiality of your relationship with your doctor and other caregivers, and the sensitive nature of your health information. We have policies and procedures in place to protect the privacy of your medical information. You will be given an opportunity to receive a copy of our Notice of Privacy Practices that describes these policies. If you have questions, please ask.

Follow-up care: The ultimate success of your treatment often depends on your efforts to follow medication, diet, and therapy plan. Your family or other healthcare professionals in the community may need to help you in your follow-up care. You have a responsibility to follow your doctor’s care plan following treatment.

Help with your bill and filing insurance claims: Our staff will file claims for you with your healthcare insurers, including Medicare and Medicaid. Healthcare billing can get quite confusing, if you have questions about your bill, feel free to contact our business office, however, if you have questions about your insurance coverage or health plan, please contact our insurance company directly. If you do not have health coverage, our financial counselor will try to help you and your family find financial assistance or to make other arrangements.

This is but a brief summary of how we view our relationship. Bottom-line is we are here to serve you. Our mission is to deliver quality healthcare in a manner that will make us your chose of healthcare provider and to do so with utmost honesty and integrity. As this is a partnership we invite your comments, questions, suggestions, and concerns for they often initiate improvements in the way in which we deliver care and do business.

Thank you for your time

Jim Jordan
